Maryland OCC 1216C Seizure Medication Authorization

Maryland child care programs use this form, without OCC 1216, when a child has a seizure, convulsion or epilepsy disorder and may need medication. The health care provider completes page 1 with the child's seizure types, triggers, emergency protocol, care after a seizure and the medication order, and staff follow it for up to one year.

Who completes it

The authorized health care provider completes and signs page 1. The parent completes the authorization and emergency contacts on page 2, and child care staff complete the checklist, including any staff training, and the administration record.

Submitting and payment

How to submit it
The completed form is returned to the child care program with the medication.
Who pays
The form sets no fee and does not address payment for completing the plan.

Good to know

  • The seizure care table asks for each seizure type with its usual length, frequency and description, plus triggers or warning signs and significant medical history.
  • Emergency protocol checkboxes include first aid (Stay. Safe. Side.), calling 911 for transport to a named hospital, notifying parents or the health care provider, and giving emergency medication.
  • The provider states whether the child needs to leave the classroom after a seizure, what help is needed and when the child can resume regular activity.
  • The prescriber line takes an original signature or signature stamp only, and the staff checklist records who trained staff to give the medication.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is OCC 1216 needed with the Maryland seizure form?

No. OCC 1216C states it is for seizure, convulsion and epilepsy medication only and is used without OCC 1216. It includes the seizure plan, prescriber's authorization, parent consent, emergency contacts and a log of each dose with the reason it was given.

How long is OCC 1216C valid?

The authorization may not exceed one year. The provider dates the plan, and at the end of the authorized period the parent's section says an authorized individual must pick up the medication or it will be discarded.

What special considerations does the Maryland seizure plan ask about?

Besides medication, the provider can note special considerations and precautions for activities, sports and trips, describe the help the child needs after a seizure, and say when the child can return to care or resume regular activity.
