Maine Nursery School Medical Statement (Staff)

Maine requires nursery school licensees, administrators and staff who care for children to be declared free from communicable disease every two years. When someone applies to work in a nursery school, they bring this short statement to a clinician, who examines or certifies them and signs the declaration that the school keeps on file.

Who completes it

The staff member's name, address and nursery school are entered at the top, and the examining clinician signs and dates the declaration. The form's letter is addressed to the examining physician, and the rule it quotes allows a licensed physician, nurse practitioner or physician assistant.

Submitting and payment

How to submit it
The signed statement is returned to the staff member for the nursery school's records, which must show compliance with the requirement.
Who pays
The form sets no fee and does not address payment for the examination.

Good to know

  • The requirement applies to nursery schools only and is a condition for keeping the nursery school license, under 22 M.R.S. 8402 and the Child Care Facility Licensing Rule.
  • The declaration is a single sentence: the individual has been examined by the signer and is free from communicable disease.
  • The declaration must be renewed every two years, so staff return for a new statement during their employment.
  • The Office of Child and Family Services lists the form as a sample staff form; the current version is dated 05/2023.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How often do Maine nursery school staff need a medical statement?

Every two years. The licensing rule quoted on the form says each licensee, administrator and other staff member who provides care for children must be declared free from communicable disease every two years, and nursery schools must keep records showing compliance.

Can a nurse practitioner sign the Maine nursery school medical statement?

The rule quoted on the form allows a licensed physician, nurse practitioner or physician assistant to make the declaration, even though the signature line is printed with M.D. The signer adds their address and the date.

Does the Maine nursery school statement require a full physical?

The form asks the clinician to examine and/or certify that the individual is free from communicable disease. It has no exam findings section; the clinician signs one declaration that the person was examined and is free from communicable disease.
