Maine School Medication Authorization (Sample Form)

The Maine Department of Education offers this sample medication authorization for school nurses to adapt with their own school logo. When a student needs medication during school hours, the parent requests it and the prescriber completes a statement that the medication is required at school to improve or maintain the student's health, with the full order.

Who completes it

The parent completes the student details and signs the parent statement. The prescriber completes the Healthcare Provider Statement and signs it, and the school nurse signs the bottom of the form.

Submitting and payment

How to submit it
The completed form is returned to the school nurse with the medication in its original container.
Who pays
The sample form sets no fee and does not address payment for completing the order.

Good to know

  • The provider statement lists the condition, medication name, dose, route, time, beginning and ending dates, possible side effects and special instructions, with prescriber signature, phone and address.
  • Prescription medication must be in the original container showing the student name, medication, dose, route, time and provider; over-the-counter medicine needs its original clearly labeled container.
  • The parent agrees that trained staff may give medication when the nurse is absent and that unclaimed medication is destroyed per federal DEA requirements after the last school day.
  • Maine posts the template as an editable Word file with translated family versions, including Arabic, Chinese, French, Portuguese, Somali, Spanish, Vietnamese, Kinyarwanda and Lingala.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is the Maine medication authorization a required state form?

It is a sample. The Maine Department of Education provides it as a template marked for the school to insert its own logo or school administrative unit name, so a district may use its own version with the same content.

Is the Maine school medication form available in other languages?

Yes. The Department of Education posts translated versions for families, including Arabic, Chinese, French, Portuguese, Somali, Spanish, Vietnamese, Kinyarwanda and Lingala, alongside the English template. They sit with the other sample family forms, such as the annual health form and the asthma, seizure and diabetes interviews.

Can the Maine school nurse call the prescriber?

Yes. The provider statement says the nurse may contact the prescriber about the medication, and the parent statement says the school nurse may contact the health care provider or pharmacist regarding the treatment.
