Missouri MO500-3304 Caregiver and Staff Medical Exam

Missouri child care facilities keep this medical examination report on file for each caregiver and staff member, ready for review during inspections. The examiner certifies the person is in good physical and emotional health and free of contagious disease, documents tuberculosis clearance, and notes any limitations or restrictions, such as being unable to lift heavier children.

Who completes it

The patient completes the identifying information, including the child care facility where they work. A licensed physician, advanced practice nurse, registered professional nurse, or registered nurse supervised by a licensed physician completes and signs the medical report.

Submitting and payment

How to submit it
The completed form is kept on file at the child care facility for review during inspections; DESE does not collect it.
Who pays
The form sets no fee and does not say whether the employer or the employee pays for the examination.

Good to know

  • The instructions explain why the exam matters: the patient may have contact with infants through school-age children, be responsible for their care day or night, and need to lift children.
  • For TB clearance the examiner checks one option: a TB Risk Assessment form attached, a negative tuberculin skin test attached, or a dated chest X-ray or written follow-up showing the person is free of contagion.
  • Unless remarks say otherwise, signing confirms the person has no condition that might endanger children and no restrictions, such as being unable to lift children over 20 pounds.
  • A nurse who signs under physician supervision must enter the supervising physician's name, and the clinic name and address may be stamped.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What TB documentation does Missouri MO500-3304 accept?

The examiner checks one of three options: a TB Risk Assessment form attached (Missouri's form is MO-580-3015), a negative tuberculin skin test attached, or a chest X-ray or written follow-up of a previous examination showing the person is free of contagion, with its date.

Who can sign the Missouri caregiver medical examination report?

The medical report section may be completed by a licensed physician, an advanced practice nurse, a registered professional nurse, or a registered nurse working under a licensed physician's supervision. A supervised nurse must also list the physician's name.

Does the Missouri staff medical report go to DESE?

No. The instructions say the completed form should be kept on file at the child care facility for review during inspections. DESE lists it among sample forms that are kept at the facility rather than submitted to the Office of Childhood.
