Missouri MO500-3303 Child Medical Exam Report

Missouri licensed child care facilities keep a medical examination report on file for each infant, toddler and preschool child. On this one-page DESE form the examiner certifies, based on history and a physical exam within the last 12 months, that the child can participate in a child care program, and writes instructions for any specialized care the child needs.

Who completes it

A physician, or a registered nurse under a physician's supervision, completes and signs the form; when a nurse signs, the supervising physician's name is entered. The child's name and birthdate appear in the identifying section.

Submitting and payment

How to submit it
The completed report goes to the child care facility and is filed in the child's record there; DESE does not collect it.
Who pays
The form sets no fee and does not address payment for the examination.

Good to know

  • The certification states the child has no special care needs unless specified, so anything the facility must do differently belongs in the specialized care section.
  • Examples listed for specialized care include special diets, allergies, ear infections, convulsions, diabetes, asthma, behavior problems and hearing or visual impairment; extra pages may be attached.
  • DESE lists the report among its sample forms, which are not submitted to the Office of Childhood but must be kept on file at the facility.
  • School-age children use a different sample form, MO500-3313 School Age Child Health Report.
