Kentucky KDESHS005 Dental Screening Form for School

Kentucky law requires proof of a dental screening or examination for five and six year olds in their first year of public school. This one-page form records whether a screening or full exam was done, coded findings for untreated decay, treated decay and early childhood cavities, and how urgently the child needs care, so the school has the result on file by January 1.

Who completes it

The parent supplies the child's details and school. A dentist, dental hygienist, physician, physician assistant, APRN or registered nurse with training completes the findings, checks their professional affiliation and signs.

Submitting and payment

How to submit it
The completed form is presented to the school no later than January 1 of the first year the child is enrolled in public school.
Who pays
The form sets no fee and does not address payment for the screening or exam.

Good to know

  • Findings are coded with simple checkboxes: 0 for no untreated or treated cavities, 1 if present, and the same for a pattern of early childhood cavities.
  • Treatment urgency is coded 0 for no obvious problem, 1 for early dental care needed, and 2 for referral for urgent care; a comment is required when a problem is marked.
  • The screener checks whether the visit was a screening or a full exam; Kentucky accepts either one as proof for the school entry requirement.
  • The form was formerly known as OH-12, and the current version is dated 3/16/2015.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
