Good to know

  • The examiner indicates whether refraction was done with cycloplegia and records unaided and best corrected acuity for each eye.
  • Six exam areas are rated normal, abnormal or not able to assess: external exam, internal exam, pupils, stereopsis, accommodation and convergence, and color vision.
  • Diagnosis checkboxes cover normal, myopia, hyperopia, astigmatism, strabismus and amblyopia, followed by recommendations and whether glasses were prescribed.
  • An anticipatory guidance block lets the doctor note eye safety counseling, education about vision disorders and a recommendation for re-examination.
