Kentucky KDESHS001 School Employee Medical Exam

Kentucky boards of education must require a medical examination with a tuberculosis risk assessment for each certified or classified employee, including substitute teachers, before initial employment. This one-page form records the applicant's medical and surgical history, an eleven-item physical, the TB assessment and the examiner's statement that the person is free of communicable disease and fit for the job.

Who completes it

The applicant or employee supplies identifying details and the board of education they are joining. A physician, physician assistant or APRN completes the history, physical and TB sections and signs the fitness statement.

Submitting and payment

How to submit it
The examiner returns the completed form to the employee or applicant, who gives it to the hiring board of education before starting work.
Who pays
The form sets no fee and does not say whether the district or the employee pays for the examination.

Good to know

  • A printed GINA notice says the examiner must not request family medical history or genetic information, and the applicant must not provide it, so the history section covers only the applicant.
  • The TB block records whether the person is high risk, whether they were referred to the local health department, any TST or blood assay performed, and a chest X-ray date.
  • The examiner certifies the person is free of communicable disease and of physical or mental disabilities that might interfere with their duties, listing any exceptions on the form.
  • School bus drivers do not use this form; the footer directs them to the separate form TC94-35E.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Which Kentucky school employees need this medical exam?

Under 702 KAR 1:160, local boards must require the exam for each certified or classified employee, including substitute teachers, before initial employment. The exam includes a tuberculosis risk assessment. School bus drivers are covered by a different form, TC94-35E.

Why does the form say not to give family medical history?

The form quotes the Genetic Information Nondiscrimination Act of 2008: an employer may not request genetic information or family medical history from an applicant or employee. The examining provider must not request it, and the applicant should not provide it to the provider or the district.

Who can sign the Kentucky school employee medical exam?

The signature line is for a physician, physician assistant or APRN. The examiner dates the examination and signs the statement that the person is free of communicable disease and of disabilities that might interfere with their duties, except as noted.
