West Virginia ECE-CC-3C Child Care Provider Medical

West Virginia uses this one-page report to show that a person caring for children in a licensed or certified setting is healthy enough to do the job safely. The examiner records medical and mental health history, vital signs, a tuberculosis screen, vision and hearing, medications that could affect child care, and any physical, mental or public health condition that might interfere.

Who completes it

An MD, DO, PA or CRNP examines the provider, completes every section and marks which of the four credentials applies next to the signature. The provider's name and date of birth go at the top.

Submitting and payment

How to submit it
The signed report goes back to the child care provider for their licensing or certification file; the form itself carries no mailing instructions.
Who pays
The form sets no fee and does not say who pays for the examination.

Good to know

  • A Mantoux (PPD) skin test or chest X-ray is required every two years, and the form records its date and result along with vision and hearing.
  • The examiner is asked whether the person can move quickly to supervise young children, lift children or supplies, and see and hear well enough for playground supervision or driving.
  • The form asks whether the examiner is the patient's regular family physician and whether any mental health condition could affect a safe, emotionally healthy environment.
  • Children in the same programs use the separate Child Health Assessment, ECE-CC-3. The provider report is dated ECE-CC-3C (5/2008).

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How often does a West Virginia child care provider need a TB test?

Who can sign the ECE-CC-3C?

An MD, DO, PA or CRNP. The signature line asks the examiner to indicate which of the four credentials applies and to enter the exam date. The examiner also prints their name and address at the top of the form.

What conditions does the examiner have to report on the ECE-CC-3C?

Physical conditions that could keep the person from typical child care tasks, mental health conditions that could affect a safe and emotionally healthy environment, medications that might affect the ability to care for children, and any medical condition that poses a public health risk.
