West Virginia ECE-CC-3 Child Health Assessment

Children enrolled in West Virginia licensed child care need a health assessment on file, and ECE-CC-3 is the state's form for it. The clinician records growth measurements with percentiles, a system-by-system exam, immunization dates, screening results such as lead, anemia and TB, allergies, and any health problems or special care the facility needs to know about for routine care and emergencies.

Who completes it

An MD, DO, PA or CRNP completes the exam, immunization, screening and recommendation sections and signs. The top of the form holds the child's, parent's and facility's details.

Submitting and payment

How to submit it
The signed form is returned to the child care facility or school named at the top of the form.
Who pays
The form sets no fee and does not say who pays for the examination.

Good to know

  • A copy of the child's Health Check exam report, attached to a copy of the immunization record, may be submitted instead of this form.
  • The form notes that age-appropriate health services and immunizations must follow the schedule recommended by the American Academy of Pediatrics.
  • The printed edition dates from December 2004, and its immunization grid lists DTP/DTaP, polio, Hib, hepatitis B, MMR, varicella and other vaccines such as PCV7.
  • Screening tests (lead, anemia, urinalysis, TB, hearing and vision) are recorded if completed, each with a date and a normal or abnormal result.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Can a Health Check exam replace the West Virginia ECE-CC-3?

Yes. The form says a copy of the Health Check exam report attached to a copy of the child's immunization record may be substituted for it. If the ECE-CC-3 itself is used, the examining MD, DO, PA or CRNP completes and signs it for the facility.

What does the ECE-CC-3 ask the clinician to record?

Health history relevant to routine care and emergencies, food or medicine allergies, length or height, weight, head circumference and blood pressure with percentiles, a system-by-system exam including development, immunization dates, screening results, the date of the last dental exam, and any health problems with recommended treatment or special care.

Who signs the West Virginia Child Health Assessment?

The signature line is for a physician or CRNP, with check boxes for MD, DO, PA and CRNP to show the examiner's credential. The examiner also enters the medical care provider's name and address and the date of the exam.
