TRS Georgia DB-3: Physician's Report for Disability

Teachers Retirement System of Georgia members applying for disability retirement must send DB-3 to every physician who treated or diagnosed them in the last 12 months. The physician reviews the attached job description and states whether the member can perform the job, which duties they cannot perform, the diagnosis, supporting findings, prospects for recovery and treatment.

Who completes it

The member completes the top section, signs a release of all medical records including psychiatric records, and attaches their job description. The physician completes the rest: ability to perform the job, job duties, diagnosis, findings and test results, abnormalities, recovery, treatment, referrals and a signed authorization.

Submitting and payment

How to submit it
The physician sends the completed report and attachments directly to TRS by mail or fax, not back to the member.
Who pays
TRS asks the physician to bill the member, who pays any charges for the report and medical records.

Good to know

  • Include test results, office visit notes, blood tests and x-ray reports for the past 12 months; cancer claims need pathology reports and AIDS claims need HIV and CD4 results.
  • TRS may require an evaluation by an independent physician of its choice, and in that case TRS pays only for that exam.
  • Mental health treating sources use DB-5 instead; DB-4 is the member's list of disability information and DB-6 is the hospital or clinic report.
  • TRS offers DB-3 as a fillable PDF and as a Word document; the printed revision is DB-3 (0505).

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does every doctor need to complete a TRS Georgia DB-3?

Every physician who treated or diagnosed the member's condition in the last 12 months should receive a copy. The member is responsible for sending one to each physician with the current job description attached, and TRS needs a current evaluation stating specifically whether the member is disabled for the job held.

Who pays for the TRS Georgia physician's report?

The member. The form asks the physician to bill the person named on the report for any charges, and tells members that charges for medical information are at their expense. TRS pays only when it requires an evaluation by an independent physician of its own choosing.

Where does the physician send DB-3?

Directly to the Teachers Retirement System of Georgia, along with test results, office notes and other attachments, rather than back to the member. TRS guarantees the confidentiality of the information. The member's signed release on page 1 authorizes the physician to send all medical and psychiatric records.
