TRS Georgia DB-5: Psychiatrist or Counselor Report

TRS Georgia members applying for disability retirement on mental health grounds send DB-5 to each psychiatrist, psychologist or counselor who treated or diagnosed them in the last 12 months. The four-page report covers ability to perform the attached job description, present illness, past history, daily activities and a 13-item current mental status assessment.

Who completes it

The member completes the identification section and signs a release of medical and psychiatric records. The treating source completes all four pages; if a licensed social worker or psychologist completes it, the referring medical doctor must also sign.

Submitting and payment

How to submit it
The treating source sends the completed report and attachments directly to TRS by mail or fax.
Who pays
TRS asks the treating source to bill the member, who pays any charges for the report.

Good to know

  • Present illness asks for mental condition, age at onset, diagnosis, supporting symptoms including any substance abuse or violent behavior, and treatment with response, compliance and side effects.
  • Daily activities cover a typical day, interests, relationships, personal habits and current ability to concentrate, sustain pace and adapt to change in a work setting.
  • Mental status items include behavior, speech, mood, perception, thought flow and content, memory, attention, estimated intelligence, reliability of the patient's report and specific symptoms.
  • Attach progress notes and hospital discharge summaries if available. TRS may require an independent evaluation, paying only for that exam.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who signs TRS Georgia DB-5?

The treating psychiatrist, psychologist or counselor completes and signs it, and the form has separate signature lines for a physician and a mental health professional. If a licensed social worker or psychologist completes the report, the medical doctor who referred the member must sign it as well.

Who pays for the DB-5 report?

The member. TRS asks the treating source to bill the person named on the report for any charges, and tells members that charges for medical information are at their expense. If TRS requires an independent evaluation by a provider it chooses, TRS covers only that cost.
