TRS Georgia DB-6 Hospital/Clinic Report

Teachers Retirement System of Georgia members applying for disability retirement send this one-page request to every hospital or clinic that treated or diagnosed them in the last 12 months. Medical records staff, rather than the treating physician, usually handle it: the facility gathers the listed records and mails them straight to TRS, where they support the disability decision alongside the DB-3 Physician's Report.

Who completes it

The member completes the top section with identifying details, treatment and discharge dates, the institution's name, and a signed authorization releasing all medical records, including psychiatric and psychological records. The hospital or clinic follows the Instructions to Hospital/Clinic block and sends the requested records.

Submitting and payment

How to submit it
The facility sends the records directly to TRS Georgia by mail, not back to the member. A member treated at more than one hospital or clinic in the last 12 months sends a separate copy of the form to each one.
Who pays
The form asks the hospital or clinic to bill the member for any charges, and TRS states that the cost of obtaining the medical information is the member's expense.

Good to know

  • Records requested include patient history, physical, operative, radiology, lab and pathology reports, diagnostic studies, discharge summaries, surgeon's reports and surgery records for the listed treatment dates.
  • Only records from the past 12 months are requested, so older history has to come through the DB-3 Physician's Report or other attachments.
  • The member's signed authorization on the form covers psychiatric and psychological records, so facilities can include behavioral health documentation without a separate release.
  • Related TRS Georgia disability forms include DB-3 Physician's Report, DB-4 Member's List of Disability Information and DB-5 Psychiatrists, Psychologists, or Counselors Report. DB-6 carries the code DB-6 (0505).

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who fills out the TRS Georgia DB-6 form?

The member fills in the top section, including treatment dates, the facility's name and a signed release of medical information, then forwards it to the hospital or clinic. Medical records staff at that facility gather the requested records and send them to TRS. The treating physician's own opinion goes on a separate form, the DB-3 Physician's Report.

How many DB-6 forms does a TRS Georgia member need?

One for each hospital or clinic where the member was treated or diagnosed in the last 12 months. TRS asks members to send a copy of the report to every such facility, and each facility returns its own records directly to TRS for the disability retirement review.

Who pays for records requested with the DB-6?

The member does. The form instructs the hospital or clinic to bill the person named on it for any charges related to the request, and TRS states that obtaining the medical information needed for a disability retirement decision is the member's responsibility and expense.
