Georgia ERS Disability Application Part IV: Physician

The Employees' Retirement System of Georgia requires Part IV of its Disability Retirement Application from each medical provider the member lists. The treating physician evaluates, against the attached job description, whether the member is medically or physically incapable of performing the duties and whether that incapacity is likely permanent. The ERSGA Medical Board uses it to decide eligibility.

Who completes it

The member completes Sections 1 to 3: general information, physician details and a release of all medical records, including psychiatric records. The physician, hospital or clinic completes Section 4, Employee Disability Information, and signs Section 5, the certification.

Submitting and payment

How to submit it
The member submits the complete, original application packet (Parts I to V) to ERSGA; incomplete packets are returned unprocessed.
Who pays
ERSGA asks the physician to bill the patient for any charges related to the report.

Good to know

  • Attach all records supporting the diagnosis for the past 18 months: history, tests, office notes, typed imaging reports, admissions, operative notes, discharge summaries and referral reports.
  • The physician must check one required opinion: able to perform the job, unable now but may recover by a stated date, or unable and recommending disability retirement, listing the duties.
  • The employer must offer any available alternative position within 45 days, and the Medical Board considers the member's ability to perform that position too.
  • The ERSGA Medical Board may order an independent medical exam at ERSGA's expense. PSERS and JRS members use separate applications.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

Does each doctor need to complete a separate Georgia ERS Part IV?

Yes. The application checklist requires a separate physician's report from each medical provider the member lists in Part II. The member gives every provider a copy of Part IV with Sections 1 to 3 completed and the current job description from Part V attached.

What records should accompany the ERS Physician's Report?

All records that document and support the diagnosis for the past 18 months: history, copies of tests, office notes, typed imaging reports, hospital admissions, operative notes, discharge summaries and referral reports. Section 4 also asks for onset date, findings, treatment and compliance, pending tests or surgery, and referrals.

Who pays for the Georgia ERS physician's report?

The member. The Part IV instructions ask the physician to bill the patient for any charges relating to the request, and tell the member they are responsible for charges relating to the records authorization. ERSGA pays only if its Medical Board orders an independent medical exam.
