Virginia VRS-6B Physician's Report: Disability

Virginia Retirement System members applying for disability retirement need their physician or another medical professional to complete this four-page report for the VRS Medical Board. It describes the functional limitations preventing usual work, each diagnosis with onset and whether it causes or contributes, treatment and medications, expected improvement within a year, and whether the condition is likely permanent.

Who completes it

The patient's physician or other medical professional completes Parts A through C and checks the condition-specific documentation in Part D. The member files VRS-6 and VRS-6A, and the employer completes VRS-6D.

Submitting and payment

How to submit it
The physician sends the report directly to VRS by mail or fax, with the documentation listed in Part D.
Who pays
VRS does not assume responsibility for fees for furnishing the information unless otherwise specified.

Good to know

  • The patient's most recent visit must have been within the last six months, and the report asks for the date the patient became unable to work.
  • Part D lists required evidence by condition, for example EKG and echocardiograms for cardiac claims, pulmonary function studies for respiratory claims, and a functional capacity evaluation for fibromyalgia.
  • Neurological claims need a comprehensive neurological exam dated within the last six months; diabetes claims need current blood sugars or A1C and urinalysis with dates.
  • VRS-6B applies to members not covered by VSDP or VLDP; Hybrid-plan and post-1999 state employees use those programs instead. Current revision 01/19.

Frequently asked questions

Who can complete Virginia form VRS-6B?

The patient's physician or another medical professional. VRS describes the form as completed by the physician or other medical professional to describe the illness or condition that may qualify the applicant, and the medical professional signs Part C with practice details.

What documentation does VRS-6B Part D require?

It depends on the condition. Part D lists documentation for musculoskeletal, cardiac, cancer, respiratory, neurological, psychiatric, diabetes, visual, auditory-vestibular, digestive and fibromyalgia claims. For other conditions, the physician still provides consultations, radiology, lab and diagnostic studies supporting the diagnosis.

Do all Virginia state employees use VRS-6B?

No. It applies to VRS members not covered by VSDP or VLDP. Hybrid-plan members and state employees hired after 1999 are covered by those disability programs instead of VRS disability retirement.
