NY Non-MTG $1,000 or Less Prior Authorization (PAR)

For New York workers' compensation treatment or testing that no Medical Treatment Guideline addresses and that costs $1,000 or less, providers file a Non-MTG Under or Equal to $1,000 PAR in OnBoard. This request type had no paper predecessor. The claim administrator responds within 8 business days, and unresolved disagreements are decided through the Board's adjudication process.

Who completes it

The Board-authorized treating provider makes the request, and delegates with their own Board-assigned user IDs can draft it for the provider's review. The claim administrator responds in OnBoard.

Submitting and payment

How to submit it
Submit online in OnBoard through the Board's Medical Portal after a claim search, under Treatment/Testing. OnBoard assigns the type from the details entered, and paper requests are not accepted.

Good to know

  • OnBoard assigns the PAR type from the CPT code, MTG site (or None) and the other request details, rather than the provider choosing it.
  • It can be escalated in OnBoard, but disputes are resolved through adjudication, not a Medical Director's Office Notice of Resolution.
  • Each PAR needs a statement of medical necessity, with narratives, progress notes and evidence of efficacy of prior treatment where it applies.
  • Treatment above the threshold uses the Non-MTG Over $1,000 PAR instead, which gives the claim administrator 30 calendar days.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What is a Non-MTG PAR of $1,000 or less?

It is the OnBoard prior authorization request for treatment or tests that cost $1,000 or less and have no applicable Medical Treatment Guidelines. The Board lists it as a new request type, launched May 2, 2022, with no paper form before it.

How long does the insurer have to respond?

Eight business days. The PAR's due date appears on the OnBoard Active tab, and if the claim administrator does not respond within the timeframe, an Order of the Chair may be issued. Deadlines falling on a weekend or public holiday may extend under General Construction Law 25-a.

What happens if the insurer denies this Non-MTG PAR?

The request can be escalated within OnBoard, but the Board's training says it is resolved through adjudication. The Medical Director's Office Notice of Resolution process applies to guideline, DME and medication requests, not to non-MTG requests.
