NY WCB Medication Prior Authorization Request (PAR)

In New York workers' compensation, drugs on the Drug Formulary need no prior authorization, but non-formulary drugs, including medical marijuana, brand-name drugs when a generic exists, unlisted combination products and compounded drugs do. Providers request them with a Medication PAR in OnBoard, choosing a therapeutic category and entering a statement of medical necessity.

Who completes it

The Board-authorized treating provider makes the request, and delegates with their own Board-assigned user IDs can draft it for the provider to review. There is no paper form.

Submitting and payment

How to submit it
In OnBoard through the Medical Portal, choose Submit a Request, Prior Authorization, claim search, the Medication category, the details, then Attest and Submit. The insurer responds within 4 calendar days.
Who pays
If prior authorization is not obtained before dispensing, the insurer or self-insured employer may deny payment for the medication.

Good to know

  • Each medication is submitted as its own PAR, even when several are requested in one session.
  • Medication PARs replaced the Medical Portal's Drug Formulary prior authorization process and, for medical marijuana, Form MG-2.
  • Within 10 days of a Level 1 partial grant or denial, request Level 2 review in OnBoard and address the reviewer's rationale; Level 3 at the Medical Director's Office can follow within 10 days.
  • Unlike other PAR types, medication PARs can go to Level 3 after an administrative denial or directly after a Level 1 decision.

How this list is checked

Frequently asked questions

Which drugs need a Medication PAR in New York workers' comp?

A drug not on the NY Workers' Compensation Drug Formulary, a brand-name drug when a generic exists, a combination product not specifically listed, a brand-name drug when a generic with the same active ingredient comes in a different strength or dosage, and compounded drugs. Formulary drugs do not require prior authorization.

How fast must the insurer respond to a medication PAR?

Within 4 calendar days. The confirmation page tells the provider to allow 4 calendar days for the insurer to respond, and the PAR appears on the Active tab with a due date. If the insurer does not respond in time, an Order of the Chair may be issued.

Is medical marijuana requested through a Medication PAR?

Yes. Non-formulary medication, including medical marijuana, is requested with a Medication PAR in OnBoard. This replaced the earlier route for medical marijuana on Form MG-2, and the Board no longer accepts paper requests.
