NY WCB MTG Special Services Prior Authorization PAR

Some services in New York's Medical Treatment Guidelines require prior approval as special services. Providers request them with an MTG Special Services PAR in OnBoard, entering the CPT code, guideline site and reference, body part and side, then a statement of medical necessity with records attached. These requests were previously made on Form C-4AUTH, and the claim administrator has 15 calendar days to respond.

Who completes it

The Board-authorized treating provider makes the request, and delegates with their own Board-assigned user IDs can draft it for the provider's review. The claim administrator's reviewer, and a physician at Level 2, respond in OnBoard.

Submitting and payment

How to submit it
Submit online in OnBoard through the Board's Medical Portal after a claim search, under Treatment/Testing. The claim administrator responds within 15 calendar days, or 30 if it requests an IME or records review.

Good to know

  • Attach narratives, progress notes, symptoms, justification for the treatment, contraindications, and evidence of efficacy of prior treatment where it applies.
  • Requests of the same type for one patient are grouped into one PAR, and after Complete Request(s) the request details can no longer be changed.
  • A Level 1 partial grant or medical denial escalates automatically to Level 2 with the claim administrator's physician; Level 3 at the Medical Director's Office must be requested.
  • The Medical Director's Office decision on medical necessity is final but does not guarantee payment or settle whether the condition is work-related.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What did the MTG Special Services PAR replace?

Special services required by the Medical Treatment Guidelines were previously requested on Form C-4AUTH, the Attending Doctor's Request for Authorization and Carrier's Response. Since May 2, 2022, they are requested through OnBoard as MTG Special Services PARs, and paper requests are not accepted.

How long does the insurer have to respond to a special services PAR?

Fifteen calendar days, extended to 30 if the claim administrator requests an independent medical examination or records review. The due date shows on the OnBoard Active tab, and if the claim administrator misses it, an Order of the Chair may be issued.

What happens if a special services PAR is partly denied?

A grant in part or medical denial at Level 1 escalates automatically to Level 2 review by the claim administrator's physician. If Level 2 does not grant it, the provider may request Level 3 review by the Board's Medical Director's Office, whose Notice of Resolution on MTG requests is final.
