NY Non-MTG Over $1,000 Prior Authorization (PAR)

When New York workers' compensation treatment or testing is not covered by any Medical Treatment Guideline and costs more than $1,000, the provider files a Non-MTG Over $1,000 PAR in OnBoard. It replaced Form C-4AUTH for these requests. The claim administrator has 30 calendar days to respond, and a denial goes to a priority hearing for adjudication rather than to the Medical Director's Office.

Who completes it

The Board-authorized treating provider makes the request, and delegates with their own Board-assigned user IDs can draft it for the provider's review. The claim administrator responds in OnBoard.

Submitting and payment

How to submit it
Submit online in OnBoard through the Board's Medical Portal after a claim search, under Treatment/Testing. The claim administrator responds within 30 calendar days, and paper requests are not accepted.

Good to know

  • Denials of this PAR type go to a priority hearing, where disagreements are resolved through adjudication.
  • OnBoard assigns the PAR type from the CPT code, MTG site (None for care outside the guidelines) and other details entered; the provider does not choose it.
  • Each PAR needs a statement of medical necessity, with narratives, progress notes, symptoms, justification and evidence of prior treatment efficacy where it applies.
  • Lower-cost care outside the guidelines uses the Non-MTG Under or Equal to $1,000 PAR, which has an 8 business day response time.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What replaced Form C-4AUTH for costlier non-MTG treatment?

The Non-MTG Over $1,000 PAR in OnBoard. Since May 2, 2022, treatment or tests costing over $1,000 for body parts or conditions without applicable Medical Treatment Guidelines are requested online, and paper C-4AUTH requests are no longer accepted.

How long does the insurer have to respond to a Non-MTG over $1,000 PAR?

Thirty calendar days. The due date shows on the OnBoard Active tab, and if the claim administrator does not respond within the timeframe, an Order of the Chair may be issued. Weekend or holiday deadlines may extend under General Construction Law 25-a.

What happens if this PAR is denied?

Denials of Non-MTG Over $1,000 PARs go to a priority hearing, and disagreements are resolved through the Board's adjudication process. The Medical Director's Office Level 3 review used for guideline, DME and medication requests does not decide these.
