NY WCB MTG Confirmation Prior Authorization (PAR)

A New York provider who wants the claim administrator to confirm that planned treatment rests on a correct application of the Medical Treatment Guidelines files an MTG Confirmation PAR in OnBoard. The request is optional for providers, but once it is submitted the claim administrator must respond, within 8 business days. It replaced the paper Form MG-1 in May 2022.

Who completes it

The Board-authorized treating provider makes the request, and delegates with their own Board-assigned user IDs can draft it for the provider's review. The claim administrator's reviewer answers in OnBoard.

Submitting and payment

How to submit it
Submit online in OnBoard through the Board's Medical Portal after a claim search, choosing Treatment/Testing. OnBoard assigns the PAR type from the details entered, and paper requests are not accepted.

Good to know

  • You do not pick the PAR type; OnBoard determines it from the CPT code, MTG site, guideline reference, body part and whether the treatment is consistent with the guidelines.
  • Each PAR needs a statement of medical necessity; for therapeutic modalities include frequency and duration, and list any spinal levels there.
  • A confirmation request can be changed to a variance request, a step the Board's OnBoard training covers separately.
  • A partial grant or medical denial escalates automatically to Level 2, and the provider can then request Level 3, where the Medical Director's Office's Notice of Resolution is final.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
