NY WCB DME Prior Authorization Request (OnBoard PAR)

New York workers' compensation providers request durable medical equipment approval through a DME Prior Authorization Request in OnBoard, the Board's online system reached through its Medical Portal. A PAR is needed before prescribing items not on the DME Fee Schedule or listed as PAR required. The provider enters each item's HCPCS code, body part, side and guideline reference, then a statement of medical necessity with records.

Who completes it

The Board-authorized treating provider makes the request in OnBoard, and delegates with their own Board-assigned user IDs can draft it for the provider to review in Draft eForms. There is no paper form, and paper requests are not accepted.

Submitting and payment

How to submit it
Log in to the Medical Portal, open OnBoard, run a claim search, choose the Durable Medical Equipment category, complete the request, and Attest and Submit. The claim administrator responds in OnBoard within 4 calendar days.

Good to know

  • Multiple DME items for the same patient are processed as a single PAR, and each item needs a duration entered under Add Details.
  • The statement of medical necessity should cover symptoms, surgery date if any, justification, contraindications, and efficacy evaluation when use is continued or extended.
  • A Level 1 partial grant or medical denial escalates automatically to Level 2; after that, the provider may request Level 3 review by the Board's Medical Director's Office.
  • Drafts not submitted within 30 days are removed automatically, and if the claim administrator misses the deadline, an Order of the Chair may be issued.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Which DME needs prior authorization in New York workers' comp?

Items not listed on the New York Workers' Compensation DME Fee Schedule, and scheduled items that show Yes in the PAR Required column. For those, the provider submits a DME PAR in OnBoard before prescribing the equipment, attaching the clinical documentation that supports it.

How long does the insurer have to answer a NY DME PAR?

Four calendar days. The OnBoard confirmation shows the expected response time, and the Active tab shows the due date. Deadlines falling on a weekend or public holiday may extend under General Construction Law 25-a, and a missed deadline can lead to an Order of the Chair.

Is the Medical Director's Office decision on DME final?

The Board's Medical Director's Office issues a Notice of Resolution after a Level 3 review, and its determination on medical necessity is final. It does not guarantee payment and does not resolve legal questions such as whether the condition is work-related.
