Nevada OD-3 Extensive Heart Exam: Police & Fire

Nevada's heart disease presumption for police officers and firefighters under NRS 617.457 depends on periodic physical examinations. The OD-3 records the extensive heart examination: height, weight, blood pressure and overweight status, a resting EKG, a stress EKG for those 40 or older or with resting abnormalities, stethoscopic examination of the heart, and triglycerides, cholesterol and urine glucose.

Who completes it

The examining physician completes and signs the form, and the officer or firefighter signs a copy as well.

Submitting and payment

How to submit it
The examiner gives a signed copy to the officer or firefighter, who signs and submits it to the employer or organization.

Good to know

  • A stress EKG is required if the person is 40 or older, or if the resting EKG shows abnormalities, provided no contraindications to the test exist.
  • Mark each test normal or abnormal and specify abnormal findings; the examiner can recommend that the person see a personal physician about correcting specific issues.
  • The limited heart exam, Form OD-4, covers vitals, a resting EKG and the stethoscopic exam only, without the stress test or lab values.
  • NAC chapter 617 sets the frequency and minimum requirements of these exams and the duties of employers and employees.

Frequently asked questions

When is a stress EKG required on the Nevada OD-3?

The form calls for a stress EKG if the officer or firefighter is 40 years old or older, or if the resting EKG shows abnormalities, as long as no contraindications to performing the test exist. The result is recorded as normal or abnormal, with any abnormality specified.

What labs are on the OD-3 heart exam?

Triglycerides, cholesterol and urine glucose. They are recorded alongside height, weight, blood pressure, an overweight yes or no, the resting EKG, any stress EKG and the stethoscopic examination of the heart, all on one page signed by the examiner.

What is the difference between Nevada OD-3 and OD-4?

Both are heart examinations for police officers and firefighters. The OD-3 extensive exam adds a stress EKG when indicated and triglyceride, cholesterol and urine glucose results. The OD-4 limited exam records vitals, overweight status, a resting EKG and the stethoscopic heart exam.
