Nevada D-32: Request for More Chiropractic Treatment

A Nevada treating chiropractor uses Form D-32 to ask the workers' compensation insurer for chiropractic treatments beyond those already authorized. The request gives the number of treatments since the first visit, the present condition with objective findings, symptoms and complaints, the number and type of further treatments, whether the worker can work now, an expected return-to-work date, and when treatment will be completed if approved.

Who completes it

The treating chiropractic physician (D.C.) completes everything above the 'For Insurer's Action' box and signs with address and telephone. The insurer completes the bottom box, granting or denying authorization or noting other action, with date, signature and title.

Submitting and payment

How to submit it
Send the typed or printed form to the workers' compensation insurer handling the claim; the insurer records its decision on the bottom of the same form.

Good to know

  • The form asks you to type or print and provide all information requested, including the claim number, employer, date of last treatment and number of treatments since the first visit.
  • Describe the present condition with objective findings as well as symptoms and patient complaints, because the request must justify treatment beyond what was already approved.
  • Define the additional treatments and give their number, plus the date by which treatment will be completed if authorization is granted.
  • Additional physical therapy has its own companion request, Form D-33, which must include a new prescription each time.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When is the Nevada D-32 used?

When a treating chiropractor wants to provide more chiropractic treatments on a Nevada workers' compensation claim than the insurer has already approved. The D-32 asks the insurer to authorize a defined number of additional treatments and records the insurer's grant, denial or other action at the bottom.

What work status information does the D-32 require?

It asks whether the injured employee is capable of working now, yes or no, with a date. If not, the chiropractor estimates the date the employee will be able to return to work, and separately gives the date treatment will be completed if the additional visits are authorized.

Is the D-32 a fillable form?

No. The D-32 on the Division's forms page is a flat, one-page PDF dated 7/99 without fillable fields, and it instructs the chiropractor to type or print and provide all information requested. The insurer completes its action box on the same page.
