Nevada C-4: Claim for Compensation/Initial Treatment

Nevada's C-4 starts a workers' compensation claim and documents the first treatment on one page. The injured employee completes the claim on the top half at the first visit. The treating provider completes the Report of Initial Treatment below: diagnosis, treatment, X-ray findings, signs of intoxication, whether the patient should stay off work five days or more, full or modified duty, and whether the condition is job incurred.

Who completes it

The employee completes and signs the top half, which includes a medical release authorization. A treating physician, chiropractic physician, PA-C or APRN completes and signs the Report of Initial Treatment, giving degree, tax ID and the date the employer's copy was delivered.

Submitting and payment

How to submit it
The provider must complete and mail the report within 3 working days of treatment. The original stays with the treating provider, and copies go to the insurer or TPA, the employer and the employee.

Good to know

  • If the patient is advised to stay off work five days or more, give the dates; otherwise mark full or modified duty and list the specific limitations and restrictions.
  • The provider states whether, from the employee's information and medical evidence, the injury or occupational disease can be directly connected as job incurred.
  • The provider also answers whether additional medical care by a physician is indicated and whether any previous injury or disease contributes to the condition.
  • Follow-up visits are reported on the D-39 Progress Report, while the employer's own report of the injury is Form C-3.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long does a Nevada provider have to file the C-4?

The form states the report must be completed and mailed within 3 working days of treatment. The four-part form keeps the original with the treating provider, with copies for the insurer or third-party administrator, the employer and the employee, and the provider certifies the date the employer's copy was delivered.

Who can sign the C-4 Report of Initial Treatment?

The degree box lists MD, DO, DC, PA-C and APRN, so a physician, chiropractic physician, certified physician assistant or advanced practice registered nurse providing the initial treatment can sign. An original or electronic signature is accepted, along with the provider's tax ID number and address.

Is a separate medical release needed with the C-4?

Generally not. The employee section includes an authorization to share medical information about the injury, except records about AIDS, psychological conditions, or alcohol or controlled substances, which need specific authorization. The separate C-4A release was discontinued effective 9/13/23.
