Nevada D-33: Request for More Physical Therapy

Physical therapists treating Nevada workers' compensation patients use Form D-33 to request therapy beyond the visits already authorized. The request identifies the treating physician, the date of last treatment and number of treatments so far, describes the present condition with objective findings, symptoms and complaints, and defines the additional treatments sought and when they will be complete. A new prescription must accompany each request.

Who completes it

The physical therapist completes the request above 'For Insurer's Action' and signs with address and telephone, and the treating physician supplies the new prescription. The insurer completes the bottom box to grant, deny or take other action.

Submitting and payment

How to submit it
Send the form with the new prescription to the workers' compensation insurer, which records its decision on the bottom of the form.

Good to know

  • The form states in capitals that a new prescription must be provided with each additional treatment request, so get the treating physician's updated order first.
  • If the insurer denies the request, the denial box states that the treating physician will be consulted in the case.
  • Unlike the chiropractic version (D-32), the D-33 has no work capability question; it focuses on condition, treatments to date and the treatments requested.
  • It is a flat one-page PDF dated 7/99 that asks you to type or print all information requested.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Does the Nevada D-33 need a prescription?

Yes. The form states that a new prescription must be provided with each additional treatment request. The physical therapist names the treating physician on the form and attaches that physician's current prescription for the further therapy being requested.

What happens after the insurer reviews a D-33?

The insurer completes the 'For Insurer's Action' box at the bottom, checking that authorization is granted for additional physical therapy treatments, that it is denied, or other action, with the date, signature and title of the person deciding. A denial notes that the treating physician will be consulted.

What should the therapist describe on the D-33?

The present condition of the injured employee, including objective findings, symptoms and patient complaints, plus a definition and number of the additional treatments requested and the date by which treatment will be completed if authorized. The date of last treatment and number of treatments since the first visit are also required.
