The Standard SI 1284: Waiver of Premium APS

The Standard's waiver of premium packet lets a totally disabled member keep group life insurance in force without paying premiums. Its Physical Capacities Attending Physician's Statement asks the physician to document functional impairment in detail: diagnoses with ICD codes, how many hours a day the patient can sit, stand, walk and drive, lifting and handling limits, vision and hearing, cardiac class, competency, prognosis and anticipated return to work.

Who completes it

The patient completes Part A of the statement and the physician completes Part B, signing with specialty and tax ID. The employee completes the Employee's Initial Statement and authorizations, and the employer completes the Employer's Statement.

Submitting and payment

How to submit it
The physician mails the completed statement directly to The Standard's claims processing office; the packet header also lists a fax line. Processing begins when all completed forms are received.
Who pays
The statement says the patient is responsible for its completion without expense to Standard Insurance Company.

Good to know

  • The benefit is for members unable to work at all reasonable occupations suited to their education, training and experience; in most cases the person must be under 60 when disability begins.
  • If the patient has seen more than one physician for the disability, each one completes a statement. Mental health treatment also calls for the psychotherapy notes authorization.
  • Part B asks the physician to attach laboratory data, special tests, surgical reports, admitting history, discharge summaries, chart notes and narrative reports.
  • Edition SI 1284 (4/25), an 18-page packet. The Standard also lists a separate New York version, SNY 1284.

Frequently asked questions

What does The Standard's Physical Capacities APS measure?

Based on objective findings, the physician marks how many hours in a workday the patient can sit, stand, walk and drive, and whether each limit is permanent or temporary. It also grades lifting, carrying and pushing by weight, hand use, bending, reaching, keyboarding and environmental restrictions.

Does SI 1284 ask about competency?

Yes. Item 14 asks whether the patient is competent to manage insurance benefits and, if not, whether the patient is competent to appoint someone to help manage them. Item 13 asks whether assessment is complicated by emotional disorders, inconsistent findings or medication dependence.
