The Standard SI 23227: Hospital Indemnity APS

The Standard's group hospital indemnity benefits pay for hospital stays, and the claim packet includes an Attending Physician's Statement confirming the admission and treatment. The physician lists dates of service with diagnosis descriptions, CPT procedure codes and procedure descriptions, then confirms symptom or injury date, first consultation, prior similar conditions, hospital admission and discharge dates, and any surgery done or planned.

Who completes it

The insured or patient completes Section A and passes the form to the physician. The attending physician completes Sections B and C; the employee completes the Employee's Statement and authorization separately.

Submitting and payment

How to submit it
The physician mails or faxes the signed statement and supporting documents, including admission and discharge paperwork, directly to The Standard.
Who pays
Section B states the patient is responsible for obtaining a complete form without expense to The Standard.

Good to know

  • Section B asks the physician to attach supporting documentation, including the patient's hospital admission and discharge paperwork.
  • For a pregnancy-related stay the form asks for the expected delivery date; the diagnosis column is still labeled ICD9, alongside the CPT procedure code.
  • The employee's part asks for the hospital bill showing the diagnosis and number of days hospitalized. If several physicians treated the patient, each completes a statement.
  • Edition SI 23227 (2/25). New York uses a separate version, SNY 23227.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What codes does The Standard's SI 23227 ask for?

For each date of service, Section B asks for a diagnosis description with diagnosis code and a CPT procedure code with procedure description. The physician also gives the date symptoms appeared or the injury happened and the date of first consultation for the condition.

Who pays to complete The Standard's hospital indemnity physician statement?

The form states that the patient is responsible for obtaining a complete form without expense to The Standard. The insurer does not pay the physician for completing Sections B and C, so any charge is between the patient and the practice.
