The Standard SI 23226: Critical Illness APS

When an employee claims group critical illness benefits from The Standard, the treating physician confirms the covered condition on the packet's Attending Physician's Statement. Section B lists covered illnesses, from cancer, stroke and heart attack to childhood conditions such as cystic fibrosis or tetralogy of Fallot, and asks for the primary diagnosis, diagnosis date, first symptoms, first consultation and any hospitalization.

Who completes it

The insured or patient completes Section A and gives the form to the physician. The attending physician completes Sections B and C; the employee separately completes the Employee's Statement and authorization.

Submitting and payment

How to submit it
The physician mails or faxes the signed statement and supporting documentation directly to The Standard, as printed in the header.
Who pays
Section B states the patient is responsible for obtaining a complete form without expense to The Standard.

Good to know

  • Attach supporting documentation such as test results, clinical diagnoses, imaging results, operative reports, pathology reports or a detailed medical statement.
  • If the patient has seen more than one physician for the critical illness, each physician completes a statement.
  • The form asks whether the patient was treated for the same or a similar condition before, with diagnosis, dates and other providers.
  • Edition SI 23226 (2/25). New York and Vermont use a Specified Disease Benefit Claim Form instead: SNY 23226 and 23226VT.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
