Pacific Life Waiver of Premium Physician Statement

When an employee stops working because of total disability and asks Pacific Life to keep group life coverage in force without premiums, the treating physician completes this three-page statement at the end of the claim package. It covers diagnoses with ICD or DSM codes, objective findings and treatment, maximum medical improvement, detailed physical capacities, and whether the patient is disabled from their own occupation or from any occupation.

Who completes it

The attending physician completes Sections 2 to 5 after the patient's identifying details in Section 1. The employee completes the Employee Statement and the employer completes the Employer Statement, then gives the employee this physician statement.

Submitting and payment

How to submit it
The package is returned by email, fax or mail to Pacific Life & Annuity Company, Workforce Benefits Claims, with medical and hospital records supporting the total disability.
Who pays
The package sets no fee and does not say who pays for the physician statement.

Good to know

  • Section 3 grades lifting and carrying in weight bands from never to constantly, and hours at a time for sitting, standing and walking, plus bending, climbing, driving and repetitive hand or foot use.
  • Section 4 asks separately whether the patient is totally disabled from their own occupation and from any occupation, with hours per day and days per week possible in each case.
  • The physician records height, weight, blood pressure, patient progress, whether maximum medical improvement has been reached, and the American Heart Association cardiac class when relevant.
  • The waiver applies only when the employee meets the policy's requirements; the APS pages in this package carry no separate printed form number.
