Pacific Life WB-49: Accelerated Benefit Physician Form

When an employee or member insured under Pacific Life group life has a terminal illness and asks for part of the benefit early through the Accelerated Benefit Option, the attending physician completes this statement. The physician confirms whether the condition is terminal, the date it became terminal and the life expectancy in months, with diagnoses, objective findings and hospital confinement dates supporting the claim.

Who completes it

The patient completes Section 1. The attending physician completes Section 2, Employee/Retiree Medical Condition, and signs Section 3; the employee completes the claim statement and the employer submits its own Employer Claim Statement.

Submitting and payment

How to submit it
The physician returns the completed form and records by mail, fax or email to Pacific Life & Annuity Company, Workforce Benefits Claims, as printed on page 2.
Who pays
The package sets no fee and does not say who pays for the physician statement.

Good to know

  • The package cover describes a terminal illness with less than 12 months to live, while the instructions say life expectancy is typically 12 to 24 months or less depending on the certificate.
  • By signing Section 3 the physician attests that all supporting documentation behind the statement has been provided, and checks off lab results, office notes, MRIs, scans, EKGs or X-rays.
  • Diagnoses take an ICD or DSM code with description and diagnosis date, and the form asks whether symptoms came from illness or accident.
  • An accelerated payment is a one-time payment that reduces the remaining life insurance amount, so the life expectancy answer drives eligibility. Edition WB-49 04/24.
