Pacific Life WB-126: Group Accident Physician Statement

An employee claiming under a Pacific Life Workforce Benefits group accident policy has the treating physician complete WB-126, the last page of the claim package. The physician confirms the diagnosis and ICD code, the injury date and accident description, emergency room, hospital and ICU dates, any surgery or diagnostic exam, therapy prescribed, and the dates the patient was told to stop and return to work.

Who completes it

Section 1 identifies the primary insured and the patient, with the patient's relationship to the insured. The attending physician completes Section 2 and signs with specialty and tax ID; the insured separately completes the WB-125 Insured/Patient Statement.

Submitting and payment

How to submit it
The package lists email, mail and fax to Pacific Life & Annuity Company, Workforce Benefits Claims, together with hospital discharge summaries or medical records that support the claim.
Who pays
The package sets no fee and does not say who pays for completing the physician statement.

Good to know

  • The physician section asks whether the condition is related to an illness or injury, whether it is work related, and for the date of emergency room treatment if any.
  • Hospitalization fields separate ICU admission and discharge dates from regular admission and discharge dates, and surgery is marked inpatient or outpatient.
  • The fraud warning on WB-126 states it applies to the attending physician portions of the claim as well as the insured's statements.
  • The package also holds the WB-54 authorization, recommended so Pacific Life can request records directly; the patient signs it if over the age of majority in their state.

Frequently asked questions

What should be sent with Pacific Life's WB-126?

The form asks the physician to provide copies of supporting medical information based on the condition. The package instructions add hospital discharge summaries and medical records that support the claim, along with the insured's statement and, as recommended, the signed WB-54 authorization.

Does WB-126 cover a spouse or child's injury?

Yes. Section 1 has check boxes for the patient's relationship to the primary insured: self, spouse, domestic partner or child. Both the primary insured's and the patient's names and birth dates go at the top before the physician completes Section 2.
