Pacific Life WB-132: Critical Illness Physician Form

After a covered diagnosis under a Pacific Life Workforce Benefits group critical illness policy, the treating physician completes this two-page statement at the end of the claim package. The physician works through the condition that applies, such as cancer, heart attack, stroke, coronary artery disease, end-stage renal failure, major organ failure or aneurysm, answers its qualifying questions, attaches the record each section names, and adds treatment, hospital and work-status details.

Who completes it

Section 1 identifies the primary insured and the patient. The attending physician completes Sections 2 and 3 and signs with specialty and tax ID; the insured completes the separate Insured/Patient Claim Statement.

Submitting and payment

How to submit it
The package lists email, mail and fax to Pacific Life Workforce Benefits Claims, together with medical records that support the diagnosis of the covered condition.
Who pays
The package sets no fee and does not say who pays for the physician statement.

Good to know

  • Each condition names its own attachment: pathology report for cancer, EKGs and labs for heart attack, imaging for aneurysm and stroke, and a transplant recommendation or UNOS placement for major organ failure.
  • The heart attack section asks whether EKG findings were consistent with infarction and whether cardiac enzymes, troponin or CPK-MB, exceeded laboratory normal levels.
  • For stroke, the physician confirms a permanent neurological deficit, a clinical diagnosis backed by neuroimaging, and deficits present at least 30 days; transient ischemic attacks are excluded.
  • End-stage renal failure questions ask whether regular dialysis at least weekly or a kidney transplant has been recommended, and the date it was recommended.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
