Pacific Life WB-128: Hospital Indemnity Physician Form

Pacific Life Workforce Benefits pays group hospital indemnity benefits based on admissions and stays, so its claim package ends with WB-128 for the treating physician. The physician confirms the diagnosis, ICD code and diagnosis date, whether the patient was seen in the emergency room, hospital and ICU admission and discharge dates, any surgery or diagnostic exam, therapy ordered and work-status dates.

Who completes it

Section 1 identifies the primary insured and the patient, with names, birth dates and relationship. The attending physician completes Section 2 and signs with specialty and tax ID; the insured completes the separate Insured/Patient Statement in the package.

Submitting and payment

How to submit it
Pacific Life accepts the package by email, mail or fax to Workforce Benefits Claims, with hospital discharge summaries or medical records supporting the claim.
Who pays
The package sets no fee and does not say who pays for the physician statement.

Good to know

  • ICU admission and discharge dates are asked separately from the regular hospital admission and discharge dates, so both should match the hospital record.
  • The physician marks whether the condition stems from an illness or an injury, gives the accident date and description if injured, and says whether it is work related.
  • Surgery is recorded as inpatient, outpatient or none, with date and procedure; occupational, physical, rehabilitation or speech therapy is recorded with frequency or treatment dates.
  • A fraud warning just above the signature states that it includes the attending physician portions of the claim.
