Hawaii TB Risk Assessment Form G (Adults and Children)

Form G is Hawaii's department-approved TB risk assessment for adults and children under HAR 11-164.2. It asks about country of birth, time spent in countries with high TB case rates, contact with infectious TB, immune-suppressing conditions or planned treatments, and household members or visitors from abroad, with separate wording for initial and annual follow-up evaluations.

Who completes it

Medical staff (RN, LPN, MD, DO, APRN, NP or PA) review the answers and sign. The patient answers the questions, or another person provides the information and is named with their relationship.

Submitting and payment

How to submit it
Keep it with the clearance documentation. If no risk factors or symptoms are found, a licensed provider can issue the clearance on TB Document F.

Good to know

  • Form G is not a TB clearance; a licensed MD, DO, APRN, NP or PA must issue Form F.
  • Any risk factor means further testing (TST, IGRA or chest X-ray) is required for clearance.
  • Living in or visiting a high-incidence country for more than three weeks, or being visited by someone from one, counts as a risk factor; Document J lists country case rates.
  • Immune risks include HIV/AIDS, steroids for a month or longer, transplant, chemotherapy or radiation, and planned TNF-alpha antagonist therapy.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

When is Hawaii TB Form G used?

For school, child care and post-secondary students and staff, and for food handlers, the DOH TB Clearance Manual starts with Form G and the Form H symptom screen. If neither is positive, the provider can issue a clearance without testing. Health care facility workers also complete Forms G and H at annual follow-up.

Can a nurse complete Hawaii Form G?

Yes. The manual lets an RN or LPN perform the risk assessment and symptom screen and measure TSTs, and the form's reviewer line lists RN, LPN, MD, DO, APRN, NP and PA. Only a licensed MD, DO, APRN, NP or PA can interpret the results and issue the clearance on Form F.
