Hawaii TB Symptom Screen Form H

Form H is Hawaii's department-approved screen for signs and symptoms of TB disease under HAR 11-164.2. It asks how long the person has been coughing, whether they have coughed up blood, and about unexplained fevers above 100 F, night sweats, weight change and fatigue over the past year, and it feeds the clearance decision recorded on Form F.

Who completes it

Medical staff review the screen and record the assessment date. The patient answers the questions, or another person provides the information and is named with their relationship.

Submitting and payment

How to submit it
Keep it with the clearance documentation; the licensed provider records the symptom screen date on TB Document F when it supports the clearance.

Good to know

  • Form H is not a TB clearance. Any symptom means further evaluation with risk factor screening and/or TST, IGRA or chest X-ray.
  • For people with a previous positive TB test, the manual pairs Form H with a chest X-ray from the past 12 months that excludes TB disease.
  • Health care workers with a prior positive test can be cleared annually on a negative symptom screen, without repeat testing.
  • The DOH TB Clearance Manual allows an organization's equivalent symptom screen in place of Form H.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is the Hawaii TB symptom screen a TB clearance?

No. Form H states that it is not a TB clearance, and the DOH TB Clearance Manual says negative tests, Form H and Form G alone do not confer clearance. A licensed MD, DO, APRN, NP or PA must review the results and issue TB Document F.

Who needs the Hawaii TB symptom screen?

People screened for school, child care, food handler or health care facility clearance, and especially those with a documented previous positive TB test. For them, a negative symptom screen plus a recent chest X-ray that excludes TB, or a negative annual screen for health care workers, supports clearance.
