Massachusetts School Vaccine Medical Exemption Letter

Massachusetts posts no form for medical exemptions from school vaccine requirements. Under 105 CMR 220.000, a medical exemption is a statement from the student's physician that a required vaccine is medically contraindicated for that student. Clinicians meet it when a family needs documentation for child care, preschool, school or college entry, and the school keeps it with the student's immunization record.

Who completes it

The student's physician writes and signs the statement, naming each vaccine that is contraindicated. A religious exemption is a separate statement written by the parent or guardian, or by the student if 18 or older.

Submitting and payment

How to submit it
Give the signed statement to the family for the school nurse or the child care, camp or college health office. It must be renewed every year at the start of the school year.

Good to know

  • DPH describes a contraindication as the reason a person cannot medically receive the vaccine. Exemptions can cover one vaccine or several; many exempt students are otherwise fully immunized.
  • For kindergarten through grade 6, DT is accepted in place of DTaP only with a letter stating a medical contraindication to DTaP.
  • Requirements apply in every grade, including students over 18 and visiting or exchange students. Local school districts enforce them, and DPH tells schools to take enforcement questions to legal counsel.
  • If a vaccine-preventable disease case occurs, students who do not meet school requirements may be excluded under DPH's exclusion guidelines for school settings.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is there a Massachusetts medical exemption form for school vaccines?

No. DPH publishes no template. Its school immunization requirements define a medical exemption as a statement from a physician that a vaccine is medically contraindicated for the student. The physician writes it as a letter, and it goes to the school nurse or program that keeps the student's immunization records.

How often must a Massachusetts medical exemption be renewed?

Every year. DPH's 2026-2027 school immunization requirements state that medical exemptions must be renewed annually at the start of the school year. Religious exemptions, written by the parent, guardian or adult student, should also be renewed annually at the start of the school year.

Which students do Massachusetts school vaccine requirements cover?

Children in child care and preschool, students in kindergarten through grade 12, and in college all full-time undergraduate and graduate students under 30 plus all full- and part-time health science students. A physician's statement of contraindication is the route to a medical exemption in each of these settings.
