NC DHHS 3995: Physician's Medical Exemption Request

When a North Carolina patient has a reason not to receive a required vaccine that is not among the contraindications listed on DHHS 3987, the physician uses this one-page request instead. It names each vaccine, describes the contraindication and proposes how long the exemption should apply. The State Health Director reviews it with the supporting records and may grant or deny it.

Who completes it

A physician (MD or DO) licensed in North Carolina completes and signs the whole form, which records the patient, parent or guardian, home county and the child care, school, college or university.

Submitting and payment

How to submit it
Mail the form with supporting documentation and the current immunization record to the NC DHHS Immunization Branch in Raleigh for the State Health Director's review. Keep a copy and give one to the person requesting the exemption.

Good to know

  • Unlike DHHS 3987, this form does not create an exemption on its own: it is a request under G.S. 130A-156 that the State Health Director may grant or deny.
  • Supporting documentation is part of the request: the instructions call for clinic notes, labs or other records that support the contraindication, plus the most current immunization record.
  • For each vaccine marked, describe the contraindication and the proposed length of time it should apply, attaching extra pages if the space runs out.
  • Vaccines on the form: DTaP, Tdap, DT/Td, MMR, varicella, IPV, hepatitis B, Hib, MenACWY and PCV13. The current edition is dated August 2020.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

When should a physician use DHHS 3995 instead of DHHS 3987?

Use DHHS 3995 when the reason for exemption is not one of the ACIP contraindications adopted under 10A NCAC 41A .0404 and printed on DHHS 3987. Listed contraindications are certified on DHHS 3987 and go straight to the school; anything else needs this request and a decision from the State Health Director.

Who decides a North Carolina physician's request for medical exemption?

The State Health Director reviews the request, its supporting documentation and the immunization record, and may grant or deny the exemption for the requested vaccines. The physician should keep a copy for the patient's file and give a copy to the person who asked for the exemption.

Can an out-of-state doctor submit DHHS 3995?

No. The form states that a physician (MD or DO) licensed to practice medicine in North Carolina must complete and sign it, and it is designed for NC-licensed physicians to request exemptions for contraindications that the NC Commission for Public Health has not adopted.
