OWCP Medical Authorization Request (WCMBP Portal)

Before rendering procedures, testing, therapy, durable medical equipment and other services to claimants under FECA, the Energy program (DEEOIC) or Black Lung, providers may need OWCP authorization. Requests are entered in the Workers' Compensation Medical Bill Process (WCMBP) Provider Portal, which shows immediately whether authorization is required and starts review on submission. Fax or mail requests on OWCP templates are also accepted.

Who completes it

The treating provider, who must be actively enrolled in the program covering the claimant, enters the service lines and attaches supporting medical documentation. The claimant completes nothing.

Submitting and payment

How to submit it
Direct data entry in the portal is recommended. Fax or paper requests must use the correct program template, can add up to 24 hours while they are uploaded, and are returned if the template is wrong or information is missing.
Who pays
OWCP reimburses only enrolled providers, and billing and payments run through the same Medical Bill Processing Portal.

Good to know

  • Enrollment is program-specific: a provider enrolled for DFEC (FECA) cannot submit an authorization for a DEEOIC claimant until enrolled with DEEOIC. Enrollment uses form OWCP-1168.
  • Confirm the claimant is eligible for the services before submitting; the portal has a tutorial on verifying eligibility.
  • DFEC templates cover general medical, DME, PT/OT, transportation, unspecified J-codes, surgical packages and home health; DEEOIC uses Forms EE-22 to EE-32, and Black Lung the CM-893.
  • For DFEC telehealth, whether authorization is needed depends on provider type and procedure codes. Opioid authorizations use Form CA-27 in the same portal.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is there a paper form for OWCP medical authorization?

Yes, as templates. DFEC has templates for general medical, durable medical equipment, PT/OT, transportation, J-codes, surgical packages and home health, and DEEOIC uses Forms EE-22 through EE-32. Faxed or mailed requests not on the correct template are returned for resubmission.

Do I need to register to get the OWCP authorization templates?

No. OWCP says no registration is needed to access the authorization templates, which are on the portal's Forms and References page, selected by program. Submitting through direct data entry, however, requires active enrollment in the claimant's program.

How do I know if a service needs OWCP authorization?

Enter the request in the WCMBP Provider Portal: OWCP says the portal instantly confirms whether authorization is required and, if it is, starts the review at submission. Faxed and mailed requests can take up to 24 hours longer to reach review.
