FECA CA-27: Opioid Authorization Request and LMN

When a federal employee's FECA claim involves opioid prescriptions that go beyond the initial fill limits, OWCP needs the prescriber's certification and letter of medical necessity before it authorizes more. The prescribing physician completes this request inside OWCP's Web Bill Processing Portal, and OWCP uses it to decide whether to keep authorizing the opioid for the claimant.

Who completes it

The prescribing physician completes the whole form, including the certification and the letter of medical necessity. The claimant and the employing agency have no section on it.

Submitting and payment

How to submit it
Registered providers log in to the OWCP Web Bill Processing Portal through the Provider link beside the FECA oval and complete it under LMN Documents. Unregistered providers use the Web Registration link first, and providers not yet enrolled download the enrollment form from Forms & Links.
Who pays
Neither the form nor the FECA forms page sets a fee for completing it.

Good to know

  • DOL posts only a sample PDF of CA-27; the working form exists for registered providers inside the Web Bill Processing Portal, under the LMN Documents menu.
  • CA-27 and CA-16 are the only FECA forms that DOL's forms page does not offer for printing.
  • OWCP requires it to authorize opioids beyond the initial fill limits, so the trigger is a continuing opioid prescription for the claimant rather than the first fills.
  • Companion forms listed with CA-27 are CA-20, the Attending Physician's Report, and OWCP-1500.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can I download a blank CA-27?

No. DOL's FECA forms page says CA-27 is only available to registered medical providers who log in to the OWCP Web Bill Processing Portal. The PDF on dol.gov is a sample. In the portal, providers click the Provider link next to the FECA oval, sign in, and choose LMN Documents in the left menu.

What if my practice is not registered in the OWCP portal?

After clicking the Provider link, unregistered providers use the Web Registration link to set up access. Providers who are not yet enrolled with OWCP at all go to Forms & Links in the top menu of the portal home page and download the Provider Enrollment form and its instructions.

When is Form CA-27 required?

OWCP requires it before authorizing opioid prescriptions for a FECA claimant beyond the initial fill limits. The prescribing physician certifies that continued opioid treatment is medically necessary and submits the letter through the portal so OWCP can decide on authorization.
