CA-16 Authorization for Examination and/or Treatment

A federal agency issues this authorization when one of its employees has a traumatic injury, naming the physician or medical facility that may examine and treat the worker for up to 60 days. The attending physician completes the initial report on the reverse side and returns it to the Office of Workers' Compensation Programs (OWCP), which pays for the authorized care under the FECA program.

Who completes it

Authorized employing-agency personnel issue the form and complete the authorization for the specific injury. The attending physician named in the authorization completes the attending physician's report on the reverse side.

Submitting and payment

How to submit it
The physician returns the completed report to OWCP or uploads it through ECOMP, and sends bills to OWCP's medical bill processing contractor.
Who pays
OWCP pays for the authorized examination and treatment under the OWCP fee schedule.

Good to know

  • There is no public download: DOL's FECA forms page says CA-16 is available only to authorized employing-agency personnel through the Agency Query System (AQS) or ECOMP.
  • The employing agency issues a separate CA-16 for each injury, so there is no blank copy for a clinic to keep on hand and fill in later.
  • The authorization is limited to examination and treatment of a traumatic injury and covers a period of up to 60 days.
  • Companion FECA forms are CA-1, CA-17 (Duty Status Report), CA-20 (Attending Physician's Report) and OWCP-1500.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Where can a physician get Form CA-16?

It cannot be downloaded. DOL's FECA forms page says CA-16 is only available to authorized employing-agency personnel, who obtain it electronically through the Agency Query System (AQS) or ECOMP. A clinic treating a federal employee without one can ask the employing agency's workers' compensation personnel, the other route the page lists.

How long does a CA-16 authorization last?

The employing agency's written authorization covers examination and treatment for a traumatic injury for up to 60 days. It names the specific physician or medical facility authorized to provide care, and the attending physician's initial report goes on the reverse side of the same form.

Is CA-16 one of the FECA forms I can print?

No. The FECA forms page says all of the Federal Employees Program's online forms can be printed except CA-16 and CA-27. Forms such as CA-17 and CA-20 are posted for printing and can be filled electronically.
