Kansas TPOPP: Transportable Physician Orders (POLST)

Kansas and Missouri share this POLST-paradigm form, distributed by the Kansas Medical Society and the Center for Practical Bioethics. It turns the goals and preferences of people with serious illness or frailty into portable orders for CPR, medical interventions and nutrition, that apply whether the patient is in a hospital, a skilled nursing facility or living independently.

Who completes it

A physician or physician assistant signs the orders when the form is completed in Kansas; in Missouri, a physician must sign. The orders reflect a conversation with the patient about goals and preferences.

Submitting and payment

How to submit it
Nothing is filed with the state; the patient keeps the completed form.

Good to know

  • Users agree not to alter the form, except to add provider or patient identifiers, a UPC or a barcode in the blank upper-right space on page one.
  • KMS recommends completing its training slides first, then reviewing the TPOPP Guidebook, before using the form.
  • The form is distributed as an Adobe Acrobat shared link rather than a direct PDF download.
  • TPOPP is not a statutory state form, and Missouri's Outside the Hospital DNR remains a separate document.

Fill it with Vero

If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.
