Kentucky MOST Form: Scope of Treatment Orders

Kentucky's standardized medical order, set by KRS 311.6225 and 201 KAR 9:470, records a seriously ill patient's choices on CPR, medical interventions, artificially administered fluids and nutrition, and antibiotics. Clinicians complete it with the patient, surrogate or responsible party, and the order then travels with the patient across settings. The Board of Medical Licensure posted an updated fillable version in July 2024.

Who completes it

The patient's physician discusses the options, signs Section D and attests that the orders match the patient's condition and preferences, while a licensed health care professional attests to reviewing any advance directive. The patient, surrogate or responsible party also signs.

Submitting and payment

How to submit it
Nothing is filed with the state. The original is the patient's personal property and goes with the patient on transfer or discharge; fax, paper and electronic copies are legally valid.

Good to know

  • Since July 1, 2022, printed and electronic versions are equally valid; a printed form must be a single double-sided 8.5 x 11 sheet, in any color.
  • Review it at least annually, on admission or discharge from a facility, after a substantial change in health and whenever treatment preferences change.
  • If an enforceable living will conflicts with the MOST, the living will prevails. To void a MOST, draw a line through Sections A to D and write VOID.
  • A provider or facility that cannot follow the orders for policy or ethical reasons must arrange the patient's transfer to another provider or facility.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is the Kentucky MOST form available in Spanish?

Yes. The Kentucky Board of Medical Licensure posts a Spanish version of the MOST form, updated in July 2024, alongside the English fillable and printable form. The physician signature and the patient, surrogate or responsible party signature are both required, and e-signed documents are valid.

What if the surrogate cannot sign the original Kentucky MOST?

The form allows a workaround: when the surrogate or responsible party is not reasonably available to sign the original, a copy of the completed form bearing their signature or electronic signature is signed by the patient's physician and placed in the medical record. The basis for the orders and how consent was obtained should be documented in the progress notes.
