Kentucky TC 94-38A to G: CDL Medical Waiver Reports

Kentucky commercial drivers who fall short of the federal DOT physical standards can ask for an intrastate medical waiver, and these seven reports carry the medical evidence for it. Each covers one condition group: vision (38A), metabolic (38B), neurological (38C), cardiovascular (38D), musculoskeletal (38E), alcohol or drug dependence (38F) and mental and emotional conditions (38G).

Who completes it

The physician or specialist treating the condition completes the whole report for that condition, and an eye doctor completes the vision report (38A). The driver files the reports with the TC 94-38 Request for Medical Waiver and the MCSA-5875 Medical Examination Report.

Submitting and payment

How to submit it
The reports are not in the KYTC forms library: the driver requests the Intrastate Medical Waiver packet from the Division of Driver Licensing CDL Section, and the completed reports go back to that section with TC 94-38 and form MCSA-5875.

Good to know

  • The driver sends only the reports that match the condition behind the waiver request, chosen from the 38A to 38G series, together with TC 94-38 and the MCSA-5875 exam report.
  • The waiver is for CDL applicants who do not meet the federal DOT physical requirements but may qualify for an intrastate waiver to drive commercially within Kentucky.
  • Because the forms come only in the mailed waiver packet, a patient who needs one must contact the CDL Section first; the KYTC forms library posts instructions, not the blank reports.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How do I get Kentucky form TC 94-38A or the other medical report forms?

They are not available through the KYTC forms library. The CDL applicant contacts the Division of Driver Licensing CDL Section to request the Intrastate Medical Waiver packet, which includes the condition-specific medical report forms. The patient then brings the relevant report to the treating physician or eye doctor to complete.

Who completes the Kentucky CDL vision waiver report (TC 94-38A)?

An eye doctor completes the vision report. The other reports in the series are completed by the physician or specialist treating that condition. The finished report is sent to the CDL Section with the driver's TC 94-38 Request for Medical Waiver and the MCSA-5875 Medical Examination Report.

What is a Kentucky intrastate CDL medical waiver?

It is a route for commercial driver license applicants who do not meet the federal DOT physical requirements but may still qualify to drive commercially within Kentucky. The driver requests it on TC 94-38 and supports it with the matching medical reports from the TC 94-38A to G series.
