Kentucky TC 94-182: Medical Review Affidavit

Physicians, law enforcement, other officials and citizens use this one-page affidavit to report a driver with a physical or mental impairment to Kentucky's Medical Review Board. The reporter explains in detail why the driver can't drive safely, including unsafe driving seen and the date of the last known seizure, and the Board then opens a case and sends the driver medical forms.

Who completes it

The reporter completes the whole form: driver information, a detailed explanation, and their own details and signature. A physician signs as a listed official, while citizens need notarized signatures from at least two people.

Submitting and payment

How to submit it
Mail, email or fax it to the Medical Review Board Office in Frankfort, using the contact details on the form.
Who pays
No fee is listed.

Good to know

  • Under 601 KAR 13:090, physicians, law enforcement, judges and other listed officials are exempt from the notarized two-citizen signature requirement.
  • Anonymous reports are not accepted, and the affidavit cannot be kept confidential: the Cabinet may have to release it to the driver or their representative.
  • After a case opens, the driver must have the medical forms completed by a treating physician; the Board decides within about 20 business days of receiving them.
  • Drivers with a seizure condition must be seizure-free for 90 days to obtain or keep driving privileges.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does a physician need a notary for Kentucky's Medical Review Affidavit?

No. Under 601 KAR 13:090, physicians are among the reporters exempt from notarization, along with law enforcement officers, KSP license examiners, judges and certain other officials. Citizens who aren't in those groups must include notarized signatures of at least two people.

Will the driver see my Kentucky Medical Review Affidavit?

Possibly. The form states that the Transportation Cabinet may be required to release it on request by the driver or the driver's representative, so it cannot be kept confidential. Anonymous reports are not accepted.
