Kentucky TC 94-202: Driver Vision Testing Certification

Kentucky requires a vision test before an initial permit, initial license, renewal or reinstatement, and this form records an exam done by an eye doctor or a credentialed medical specialist under KRS 186.577 and 601 KAR 12:120. The examiner certifies whether the applicant meets the visual acuity and visual field standards, and failures are reported to the Division of Driver Licensing.

Who completes it

The applicant completes Section 1 and signs a release. A vision specialist (optometrist or ophthalmologist) or a KYTC-credentialed medical specialist (physician, osteopath or APRN) completes Sections 2 and 3, the results and certification.

Submitting and payment

How to submit it
For a pass, give the original to the applicant to present when applying. For a fail, give the original to the applicant and mail or email a copy to the Division of Driver Licensing.
Who pays
The form lists no fee.

Good to know

  • Screening standard: 20/40 or better in the better eye, with fields of 30 degrees left and right and 25 degrees above and below fixation; after vision specialist referral, 20/60 qualifies.
  • Applicants who meet the standards only with corrective lenses get a corrective lens restriction.
  • The form is valid only if signed less than 12 months before the application or renewal.
  • Physicians and APRNs who aren't eye specialists must first be credentialed by KYTC (TC 94-203); applicants can appeal a vision specialist's denial to the Medical Review Board within 20 days.
