Maine MVE-103: BMV Eye Examination Form

Maine's Bureau of Motor Vehicles accepts this two-page eye examination report, revised 05/01/2023, from a licensed vision examiner when a driver applies for a license, renews at certain ages or has an eye condition. The examiner records acuity in each eye, total continuous visual field, and any defect, bioptic use or progressive condition, and sets a reexamination interval.

Who completes it

The driver fills in identifying details. A licensed vision examiner completes the rest based on the exam and Maine's Functional Ability Profile rules, then signs with the exam date.

Submitting and payment

How to submit it
The examiner returns the form to the BMV Medical Section by mail, email or fax, or the driver brings it to the license exam or renewal.
Who pays
The driver pays for an exam by a doctor of their choice; the BMV offers a vision test at no fee during the license exam or at a branch renewal.

Good to know

  • Telescopic or bioptic lenses may not be used to test acuity, and field expansion devices may not be used to test fields; a field under 110 degrees total triggers FAP review.
  • Acuity of 20/50 to 20/100 in the better eye brings a corrective lens and daylight-only restriction; diplopia can bring other clinician-recommended measures.
  • Renewal screening applies at the first renewal after age 40, every second renewal after that, and every renewal after age 62.
  • Drivers with a field defect can ask for Exceptional Case review; criteria include a stable defect present at least 12 months, 20/40 in the better eye, no diplopia and a road test.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can a bioptic be used on Maine's MVE-103 vision test?

No. Telescopic or bioptic lenses may not be used when testing visual acuity, though the examiner can check a box noting the patient uses bioptic telescopic lenses for driving. Field expansion devices are likewise not permitted when measuring the visual field.

How recent must the eye exam be for Maine BMV?

The exam date on the form must be within the past 12 months, and BMV will not accept an exam done more than one year before it receives the form. For a renewal, the outside exam may not be done more than a year before the renewal date.

What visual field does Maine require for driving?

Maine's Functional Ability Profile rules require a continuous horizontal visual field of 110 degrees. The examiner records total degrees on the form and refers to the rules when the field is under 110 degrees or there is field loss after a stroke or brain injury.
