Maine CR-24 Addendum: Narcolepsy/Cataplexy

For Maine drivers with narcolepsy, the treating clinician can supplement the CR-24 Driver Medical Evaluation with this one-page Word addendum. It captures treatment type, effectiveness and adherence, the Epworth Sleepiness Scale score, and whether cataplexy is present, how often, how predictable and what triggers it. These are the details Maine's Functional Ability Profile rules ask clinicians to document.

Who completes it

The clinician treating the narcolepsy completes and signs the whole addendum. Maine's rules note these patients are usually followed by a sleep specialist, neurologist or pulmonologist, or their nurse practitioners and physician assistants.

Submitting and payment

How to submit it
The addendum is faxed to the BMV Medical Section, with the driver's name and date of birth at the top.

Good to know

  • Maine's rules ask for the presence and severity of cataplexy, its triggers, residual daytime sleepiness and adherence to medication and behavioral strategies, which match the addendum's fields.
  • The cataplexy section asks for the date of the last event, episode frequency, whether episodes are predictable, triggers, and the circumstances and symptoms.
  • The rules recommend follow-up at least yearly, ideally twice yearly, once stable on medication; MSLT or MWT testing may be used when sleepiness poses a safety risk.
  • The addendum is a legacy Word file with no printed revision date. The same Functional Ability Profile chapter also covers idiopathic hypersomnia.

The addendum has a field for the Epworth Sleepiness Scale score, alongside the type, effectiveness and adherence of treatment. Maine's guidance stresses documenting the degree of residual daytime sleepiness when the BMV reviews drivers with narcolepsy.
