Maine CR-24: BMV Driver Medical Evaluation

Maine's Bureau of Motor Vehicles uses this two-page advisory report, revised 05/01/23, to learn about physical, emotional or mental conditions that could affect safe driving. The clinician lists each diagnosis with its Functional Ability Profile level under rules effective May 3, 2023, and can recommend restrictions, a road test or suspension. The Secretary of State decides with the Medical Advisory Board's advice.

Who completes it

The driver fills in identifying details. An appropriate medical or paramedical professional completes everything else, including the diagnosis table, comments, medications and the Certificate of Examination; the driver's signature is not required to submit it.

Submitting and payment

How to submit it
The clinician returns the completed form to the BMV Medical Section in Augusta by mail, email or fax.

Good to know

  • A clinician acting in good faith is immune from damages claimed as a result of filing the report, under 29-A MRSA Section 1258(6).
  • For loss of consciousness, seizure, stroke or hypoglycemia needing third-party help, give dates and describe the latest episode; chronic respiratory disease needs an oxygen saturation, noting room air or oxygen.
  • Hypoglycemia level 3b and prescription medication or opioid replacement level 3c need a sub-category; substance use level 3b needs the time substance-free.
  • The date of last assessment must be within the past 12 months unless BMV specifies otherwise. A narcolepsy addendum and the MVE-103 eye form supplement it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is a Maine CR-24 report a recommendation or a decision?

It is advisory. The Secretary of State determines a person's competence to drive with advice from the Medical Advisory Board, using the Functional Ability Profile levels and the clinician's comments, such as recommended restrictions, a road test or suspension, and any described deficits.

Does the patient have to sign Maine's CR-24?

No. The Certificate of Examination may be submitted without the patient's signature, and a clinician filing in good faith is immune from damages. The form also includes a driver authorization for release of medical information that the patient can sign.

Where do clinicians find Maine's Functional Ability Profiles?

On the BMV medical review pages, which post the rules effective May 3, 2023, plus short videos and PDFs by condition, such as cardiovascular disease, dementia, hypoglycemia, seizures, sleep apnea, substance use disorder and visual conditions, explaining each profile level.
