Montana 20-1900: MVD Driver Medical Evaluation

Montana's Motor Vehicle Division sends this four-page evaluation when its records show a driver may have a condition that could affect safe driving. The physician or provider reports history, medications, any lapse of consciousness, impairments and diagnosis, and recommends adaptive equipment, restrictions and periodic testing; MVD makes the licensing decision and also weighs non-medical factors.

Who completes it

The patient signs the release in Section 1a in the provider's presence and marks whether the credential is a Class D license or a CDL; the physician or treating provider completes Sections 2 through 10.

Submitting and payment

How to submit it
Return it to the MVD Medical Unit by fax or mail, as printed on the form's last page.
Who pays
The release the patient signs states that any expense involved is charged to the patient, not the State of Montana.

  • The August 2026 revision (8/26) asks whether MVD should run periodic road tests, knowledge tests or medical evaluations, at 6 months, 1 year, 2 years or another interval.
