Colorado DR 2498: Eye Exam for Drivers 80+ Online

Colorado drivers aged 80 or older can renew their license online only with an eye doctor's statement of a recent exam. On this one-page form the optometrist or ophthalmologist certifies that the driver meets Colorado's vision recommendations, gives the exam date and states whether corrective lenses are required, under C.R.S. 42-2-118(1.5)(a)(III).

Who completes it

The driver completes and signs the Driver/Patient Section, authorizing the eye doctor. The optometrist or ophthalmologist completes the Optometrist/Ophthalmologist Section with name, license number and state, address, exam date, lens requirement and signature.

Submitting and payment

How to submit it
Once both sections are complete, the driver uploads the form while completing the renewal application at myDMV.Colorado.gov.

The form lists no fee.

Good to know

  • The examination must be within 6 months, and you choose either corrective lenses required or corrective lenses not required.
  • The standard: 20/40 or better in either eye with or without lenses, and at least 120 degrees combined horizontal field, or 60 degrees in the other eye if blind in one.
  • If the driver doesn't meet the recommendation, use the DR 2402 eye report process instead; the driver must then renew in person at a Driver License Office.
  • DMV holds the information in strict confidence under C.R.S. 42-2-121 and the federal Driver's Privacy Protection Act.

How this list is checked

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What if my patient doesn't meet the vision standard on DR 2498?

Don't sign the certification. The form directs you to proceed with the DR 2402 Confidential Eye Examination Report and its process instead, and the driver will need an in-person visit to a Driver License Office rather than renewing online.

How recent must the eye exam be for Colorado form DR 2498?

The date of examination must be within 6 months. The eye doctor enters that date, states whether corrective lenses are required, and signs. The driver then uploads the completed form during the online renewal at myDMV.Colorado.gov.
